Question: What are the purchase requirements? 
Answer:

• Consumers who collect 7 to 11 original UPC stickers from TOMBSTONE 12” pizzas, and send them in all at one time to the fulfillment center, along with original tear pad mail-in form obtained at retail that is completely and correctly filled out, will receive a $5.00 electronic credit (“Bill-Pay reward”) to the selected utility provider indicated on tear pad mail-in form.



• Consumers who collect 12 to 16 original UPC stickers from TOMBSTONE 12” pizzas will receive a $10.00 electronic credit to the selected utility provider indicated on tear pad mail-in form.



• Consumers who collect 17+ original UPC stickers from TOMBSTONE 12” pizzas will receive a $15.00 electronic credit to the selected utility provider indicated on tear pad mail-in form.

Question:  Why is a copy of my utility statement needed?
Answer:  This is to ensure that the correct account is credited, in case the written account number is illegible.

Question:  Why is my utility provider account number needed?
Answer:  Your utility provider account number is needed in order for us to credit your account correctly. 

Question: How many times can I submit a mail in form?
Answer:  This promotion is limited to one per household.

Question: When will my utility provider receive the credit?
Answer: You will typically see the credit reflected on your utility bill within 6-8 weeks of receipt of mail- in request. 

Question: Where can I apply my bill pay credit?
Answer: Your credit can be used for bill payment towards most major home heating, mobile phone, cable, satellite and utility providers.

Question: When is the last day I can submit a mail in form?
Answer: All requests must be postmarked by 1/15/10

Question:  When does my mail-in request need to be received?
Answer:  All requests need to be received by the address above by 2/15/10 to qualify for this offer.

Question: What happens if I do not see my credit applied to my bill?
Answer: If after 2 billing cycles your credit has not appeared, please contact TPG Rewards customer service at 1-800-290-0816.

Question: What happens if my utility provider does not participate in the program?
Answer: Our system has many utility operators listed.  If your provider is not listed, we will mail you a check made out to you, in the amount that your mail-in request qualifies for.
